Extracellular vesicles (EVs), especially exosomes, have emerged as a new role in the development of Alzheimer's disease (AD) in the past 10 years, which has great potential as a diagnostic and therapeutic tool. Exosomes and other EVs are enriched with the sphingolipid ceramide as well as other more complex glycosphingolipids. At least a exosome subgroup needs neutral sphingomyelinase activity for biogenesis and secretion. Because ceramide is often increased in AD, the secretion of exosomes may also be affected.
